GLP-1 and glucagon, without GIP

Survodutide (BI 456906) is a synthetic peptide agonist at the glucagon receptor and the GLP-1 receptor. Structurally it derives from glucagon rather than from GLP-1 — the two hormones are closely related members of the same superfamily, which is what makes dual agonists of this kind chemically feasible at all.

It is acylated for albumin binding and dosed weekly. Omitting GIP is a deliberate choice rather than an omission, and it is what gives the compound its experimental value: comparing it against a GIP-containing agonist isolates the GIP contribution.

The hepatic rationale

The liver expresses glucagon receptors densely, and glucagon signalling there promotes fatty acid oxidation and reduces hepatic steatosis by a mechanism entirely separate from reduced food intake. That gives a glucagon-containing agonist a direct route to hepatic endpoints.

  • MASH. Phase 2 data on histological improvement, the endpoint that matters in this indication and the hardest to move.

  • Hepatic fat fraction, measured by MRI-PDFF.

  • Fibrosis endpoints, the distinguishing question in liver disease research — steatosis is easier to shift than fibrosis.

  • Energy expenditure, the systemic counterpart of the hepatic mechanism.

The trial literature

  • SYNCHRONIZE — the phase 3 obesity programme.

  • Phase 2 MASH, reported in 2024, with histological endpoints.

  • Phase 2 obesity and type 2 diabetes, establishing the dose range.

Survodutide is not approved in any jurisdiction. Its glucagon component also makes glycaemic control a variable to watch rather than assume, which is a live question in the ongoing programme.

Handling

Supplied as a lyophilised powder. Reconstitute gently; store reconstituted material refrigerated and protected from light.

Read alongside

Mazdutide is the other GLP-1/glucagon dual agonist here and the direct structural comparator. Retatrutide adds GIP to the same pairing, and Semaglutide is the GLP-1-only baseline.

Documentation and verification

Certificates of Analysis are published for tested products and batches. A certificate is a batch-specific record: it reports what was measured, by which method, on a stated date, against a batch identifier. It is evidence about the sample analysed at the time of analysis, and it is silent on attributes it does not list and on what has happened to the material since.

The batch number is the field that connects the document to a physical vial. Where the identifier on a vial does not appear on a certificate, the certificate does not describe that vial, however thorough the analysis behind it was. Batch records can be searched directly, and the guides below explain what each field establishes and what it does not.
